For privacy requests, describe the record or email list involved using only enough detail to identify it. Use the email associated with it where possible. Do not send passwords, identity documents, or repeat private application answers. Staff will verify your request before making changes.

Fields marked * are required. Your name, email, and message are stored for authorized administrators to respond to and archive your inquiry. Inquiries are kept for 12 months from submission and deleted at the next monthly review, including archived records and notification email copies. Archiving does not delete your information or restart the retention period. Do not include confidential information. Sending this form does not add you to the optional mailing list.
